How to fill in and submit the Intelysis, RCMP Vulnerable Person clearance form:

Fill in the application form with the information requested.

• Make sure that you write in Eldercare Home Health Inc. near the bottom of page 2 under “Name of Organization”. By doing so you are giving Intelysis permission to contact us directly with the results of their search. The search is part of your application to work with Eldercare Home Health Inc. Successful completion of the search is not a guarantee of employment.

• If you have access to a fax and are paying by credit card you can fax in your forms.

• If paying by cheque, make a note that you are including a cheque.

• If you do not have an e-mail, make a note in the e-mail address area for them to send your copy of the results of the search to your home address.
