Eldercare Home Health Inc. has been providing care exclusively to the elderly since 1995. We work with all the major hospitals in Toronto and are a trusted resource to Social Workers and Discharge Planners who work with the elderly. Eldercare Home Health was founded by Lisa Wiseman. Lisa is a Registered Nurse with a Baccalaureate degree in Nursing from the University of Toronto. Lisa has worked in management at Queen Elizabeth Hospital and The Baycrest Centre for Geriatric Care, two of Canada's leading centres for geriatric care. Lisa started Eldercare Home Health because she and her colleagues in Gerontology recognized the need for a home health care provider that would take a holistic approach to providing Elder Care and specialize in caring for the unique needs of the elderly. Early in her career Lisa worked as a home care nurse and as a front - line nurse in some of Canada's foremost geriatric hospitals. Eldercare Home Health works with Power of Attorneys as well as directly with clients and their families. Our caregivers are bonded and insured an supervised by an RN. An RN is on call 24/7, every day of the year for emergencies. We take a holistic approach to providing care acting as case managers, advocating on behalf of our clients. We can co-ordinate our care hours with those offered by the government homecare program. We enjoy a very positive relationship with many of the CCAC offices in the GTA. A no-charge, no-obligation in-home/institution assessment is the first step in determining client's care requirements and can be arranged by phone, usually for the next day. Eldercare Home Health's rates are competitive. There is no additional fee for the involvement of the RN supervisors.
