"Way back, as a 15 year old volunteer working with seniors at the Baycrest Center for Geriatric care here in Toronto, I never imagined that one day I’d be the founder and President of a company dedicated to providing Registered Nurse supervised and case managed care to seniors."

A few years after that initial experience at Baycrest I graduated from the University of Toronto’s Nursing program and went to work as a visiting Nurse. Later I joined the management team at The Baycrest Centre for Geriatric Care and eventually ended up at the Toronto Rehabilitation Institute.

It was at the Toronto Rehabilitation Institute that I really became aware of the “boomerang effect” – we’d work with patients to help get them back on their feet, they’d return home, and then sure enough, a few weeks later end up back on our doorsteps.

In consultation with my colleagues it became very clear that there was a real gap in the care picture – and the gap was at home.

Registered Nurse

Lisa Wiseman’s Bachelor of Science in Nursing degree from the University of Toronto. Lisa is a past President of The Gerontological Nursing Association – Toronto Chapter. Lisa has also been involved with the Regional Geriatric Program, a program that coordinates the services of hospitals to better provide senior care. Lisa is Gerontological Nurse Certified (Canada) GNC(C), and is also Hospice Palliative Care Nursing (CHPCN(C) certified through the Canadian Nurses Association.

I started Eldercare Home Health to fill that gap, to provide a level of care that would help minimize our clients’ need to return to hospital.

Since then we’ve broadened our service and we now work with clients who are at home, in retirement homes and long term care facilities (Nursing Homes) and in Hospitals.

Our model is unique, we provide Personal Support Worker (PSW) care that is actively case managed and supervised by Registered Nurses – at no additional charge.

Basically, we provide the kind of care that your parent really needs – arranging caregivers, liaising with doctors and pharmacists, helping to arrange for equipment and supplies, advocating, accompanying your parent to appointments, managing medications etc., and we keep things as simple as possible – we bill only for the time the caregiver(s) are on site, and we do not bill for the involvement of the Registered Nurses.

This way if you’ve got questions or need advice etc., you never have to be worried about calling the Nurse who’s case managing your parent’s care. And the Nurse can make the visits with your parent that they need to, without having to worry about adding to your care costs.

While I’m the face of the company, we have a terrific team of Nurses, PSW caregivers and dedicated support staff who help make sure your parent gets the care they deserve, and keep things running smoothly.
